Perhaps reports of success were a bit premature:

On my W2000 boxes (which may be a clue?) when SCHPAT chains back to itself it still terminates in an error in ashw32.exe.

If you tell me what specific traces to add and where to trap this issue i will and get you a log as soon as possible.

Jack.. i just got it to crash on my XP box as well.. i sent you a fresh log with a bunch of traces.. hope it helps.
